Your first visit marks some very important steps in our doctor patient relationship.
First, we obtain important background information including your medical history, so we can understand where you have been. Before your visit, you can easily print out the New Patient Forms to fill out in advance at your convenience and bring with you to your appointment.
Most importantly, we give you time to get to know us. You can ask questions and establish a comfort level with your doctor. There’s also background information about our committed staff and our first visit procedures.
Please see the practical information you need, such as a map and directions to our office, practice hours, payment policies and more.
